Very interesting and disturbing investigation. Just as was the case with the Piper Alpha disaster, there are going to be a great deal of changes brought about by this event. One of which, I hope, will be a significant change in the way in which safety is approached in the future. The present approach to implementation of safety policy is frequently misguided, driven, as it is, by a lack of management understanding of what is important.
